Transition 6 Program
The Transition 6 Program or “T-6” at Tanglewood Middle School consists of thirty 6th grade girls. These young ladies are provided with services and engaged in activities that are geared towards transitioning them into middle school and will follow them through eighth grade. The students will be involved in setting goals, academic enrichment, developing skills and hobbies, field experiences to local businesses and colleges, and team building activities.
The case manager assigned to the site is responsible for working with the students and their families by assessing their needs, goals, achievements, and struggles throughout the school year. Students who qualify for the “T-6” Program are identified by the case manager, school administrators, and teachers. The qualifications may include, but are not limited to the following: students who are having challenges academically; those with a need for organizational skills and goal setting; and those who possess a desire to succeed and put forth effort to improve previously identified areas of concern.
At the end of the year each young lady who participates in the “T-6” Program will be expected to have set long and short term goals, and to have taken steps towards achieving personal success. Actions may include: improving Math and English/Language Arts scores; developing better organization skills; increasing knowledge of middle and high school requirements; and achieving promotion to next grade level.
As with all CIS programs, the focus is on the student as a whole ensuring that any barrier to educational success is addressed by surrounding students with a community of support, empowering them to stay in school and achieve in life.
